Facilitated Support Program
The Facilitated Support Program works in partnership with people with disability and their circles of support to assist people to live the lives they want to live with the services and supports they need.
The Facilitated Support Program is funded by the NSW Department of Family and Community Services and assists people who were gaining support through Ageing, Disability and Home Care to prepare for the rollout of the National Disability Insurances Scheme in our area in July 2017.
Facilitators work in partnership with people with disability to identify and document their hopes and dreams for the future and their support requirements. Facilitators work with people to build their formal networks and informal circles of support using a strengths based, capacity building and person centered practice model.
Facilitators support people to feel more connected, become more involved in community activities, have more choices, feel more in control of their lives and to prepare for the future.
Access to the Facilitated Support Program is limited to referrals from Ageing, Disability and Home Care.
