Key Transition Points Mapping Project
Mid Coast Communities is working with the NSW Department of Family and Community Services (FACS), the District Child and Family Planning Group and local Child and Family Inter-agencies to map services and programs that families might access when their child/children “transition” from one life stage to another such as at birth, in early childhood and when starting school.
The Key Transition Point Mapping Project will identify where services and supports are available and located in our Mid North Coast Local Government Areas (LGAs) of Coffs Harbour, Nambucca, Bellingen, Kempsey and Port Macquarie.
When finalised, the Key Transition Points Maps will be available on our website and will assist FACS to identify and respond to gaps or duplication in each LGA and assist Community Services as a directory and for planning purposes.
